Questionnaires aren't generic - they are carefully crafted to understand consumer preferences and decision-making for specific target audiences and for specific goals. If this is a school project, then I suggest crafting your own questionnaire. If this is a professional need, then hire an expert to help you create a questionnaire that would address your specific goals.

And? You have a need, which is fine. But what you're not telling us is … everything.

What KIND of survey? Aimed at whom? To accomplish what? To furnish you with what kind of data? To be used how? By whom? over what period? In what manner?
