Sample Website User Surveys?

I need help conducting an online survey for my website. The survey must not only cover demographics, but also consumer purchasing behavior, as I am shopping around for potential sposors. Does anyone know where I can access sample surveys online? Any help would be greatly appreciated- Thanks!

    The folks at www.questionpro.com do a great job. We've also used www.zoomerang.com in the past.

    My fast tips would be:
    - think carefully about your questions
    - pre-test your questions with a few people in the office
    - focus on asking something you can ACT ON, once you have the answer
    - keep it SHORT
