Release Form For User Story

I am writing User Stories on our product that may be used on the web, in brochures and in advertisements. They will include copy and photographs. Is there a standard format for a release form, that is legally binding, for the user to sign to ensure that I can use the information and photographs in all these marketing tools?

Where can I find examples of these forms?
