Can You Give Me Some Advice About This Questionnaire?

Hello, everyone, now I really need your help. I designed the questionnaire below. Can you give me some advice about how to improve it?

The questionnaire

The bear factory opened the first shop in 2000, four years later, now it has been one of the most popular toyshops in UK. In order to improve our service and satisfy more children, we really need you helps. Please fill in the questionnaire and let us know what do you want.

    How about that, I live in the UK with kids and I have never heard of "Bear Factory", you even have one 40 miles away in Nottingham!

    Back to your questionnaire:

    Firstly ensure that you have your objectives from the questionnaire known, this can include some or all of these:

    - The potential market for a new product or service
    - Ratings of current products or services
    - Employee attitudes
    - Customer/patient satisfaction levels
    - Corporate images

    I realise that some of these are answered in the questionnaire, but its worth checking.

    Also remember that questionnaires can be used to find out about peoples attitude, awareness, intention and behaviors. In a socialty today where loyalty is fairly low with most businesses and markets intention and behaviour data is vital.

    Kinda based on Jetts new question 6 above (which I like btw), it may be better phrased as:

    How important are the following to you:

    [Very] [Important] [Unimportant] [Very
    Important]
    Service
    Store Size
    Product Selection
    Price

    Then ask:

    On a scale of 1 - 10 how would you rate Bear Factory:

    ...........................[score]
    Service
    Store Size
    Product Selection
    Price

    This way you find out what is important and how you score against that, for example if Store Size isn't important, why spend more on getting a bigger location! You always need to know what your scores mean in relation to the customers expectations.
