Is there an easy to use online marketing request form? My company gets a number of request from our field staff for marketing projects. I would like to centralize these requests into one form that can be accessed through our corporate portal. Often times the field will include attachments with their email request. Unfortunately, if I put it behind a login page I'll have few users. Also, having our IT group enable SSO will take time.

Does anyone use something inexpensive that's proved effective? At a minimum I could setup a word doc, but I wanted something that could also capture attachments.
