Does Anyone Have A Good Brief Template

I am developing a new brief template for the whole company to use to submit to the marketing department. This brief is to use on any material that needs to be developed by marketing that is not part of the approved communication plan. I require a detail template that is simple enough to use for all.

    Handed over to creative on
    Concept expected by
    Order Date
    Parent Company
    Brand
    Positioning Statement of Brand / Baseline
    Target Audience
    Target Markets
    Personality of The Brand
    Brand communication for this specific campaign
    Functional Benefits of the Brand (product)
    Kindly provide advertising material to aid conceptualization (TVC, Radio, Print – all 3). This is most important.
    Activity
    Activity Duration (weeks)
    Purpose of Communication (Launch, Seasonal discount, etc.)
    Giveaways / Presents that can be given out
    Contact Person & Details

    Who is the contact person
    Expected timeframe
    What product or product line
    Expected medium or media
    Specific audience target
    What they think they want from marketing
    Helpful notes and/or known issues

    That brief and simple ramp outline should be enough to get the marketing contact up to highway speed. At this point there should be a phone call.
