    While not a QC expert, having run several businesses, here's how I would approach a QC problem:

    1) Identify the exact issues: What problems are you seeing? What are the claims for?

    2) Quantify the defects - How many of each?, When is the defect showing up?

    3) See if the defects could be related - group related problem areas together: i.e., look for common issues/causal factors (manufacturing process, materials, usage situations, etc.)

    4) Quantify the extent of the issue: How serious is each? How frequently do they occur?

    5) Rank the problems in terms of a) their contribution to the total, and b) their seriousness (i.e., likelihood of causing a chargeback)

    6) Attack the highest incidence problems that have the highest linkage to chargebacks first

    7) Reverse engineer the manufacturing process to identify where and when the problem could be occurring

    8) Start fixing! Try alternative processes, materials, etc. and put into accelerated use testing to determine the best solution

    You might want to hire a qualified QC expert, or at least someone with solid manufacturing experience. This doesn't sound like a marketing issue.
