Surveys And Seo

Upon adding survey to my website, i dont see the URL now in the address bar and also on testing this on crawling tool it did not give any info on the other url's linking to the site's main url. Can anyone tell why this is happening?

    What's the URL of the page for the survey?

    Without the URL it will be difficult to help.
