Lead Management/ Follow Up Process

I was wondering if anyone could share some guidance on creating a formal lead management/ follow up process.

I would like to share this with my sales team and offer clarity on how the leads will be distributed, who the leads will go to and the steps to follow up. But I am struggling to put this information in a formal template. Does anyone know of any good tools/ flowchart or templates to help with such a process? I am aware this will differ for every business but some example templates/ suggestions would really help.
