Need A Tagline For Pathway Support Partners

Pathway Support Partners provides care to young adults between the ages of 16-21 who have either been in residential care homes or prison, but then need to transition to independance, we will work with local authorities and young people to rebuild a new path way in life. I've thought of a few but would be greatful for any new ideas, here's mine.

Transition to a better future
Paving the way for young people
Leading the way for young people
Leading the way to independence
From Care to independence
The first step to a brighter future

    Who is your target audience for this tagline? What do you want them to know about the organization? What do you want them to do once they learn about you?

    Why not make it simple: Providing Transitional Care to 16-21 year olds

    The target audience would be local government authorities, private residential care homes, social workers and other care agencies.

    By the way, thanks for everyones suggestions so far..
