Name Change/branding

I am a consultant working for a hospice and home nursing company that has grown business via acquisition. Several of the "other" named acquisition agencies have now decided they will begin using the corporate name. I'm looking for a clever name change concept/campaign to deliver this information to their communities.

    What exactly do you need from us? It would seem that a simple name change might work. If there's value in the old names, it would be OK to just include "Formerly XYZ Hospice" in small print under the new name ... for 6-12 months.

    Not sure there's much more to it than that, based on what you've told us. Is there an issue I'm missing?
