Making Icons For Application And Ui

We are redesigning the UI of our software application and need some new icons. I've been on projects where great designers can't make the transition to 16 pixel x 16 pixel graphics. Are there any good books, software, or other resources that anyone knows of. I only found 2 books on Amazon, one was poorly rated and the other was more process than tactics....

    Thanks for the tips so far but we are not going to hire an independant contractor or use any stock icons.

    From everything I can see about Icon Builder it's a great tool if you already have some good looking icons built, but that's the point we're trying to get to...

    I'd really like something that talks about best practices - things to do/ not to do.
