    Some basic questions like

Do you research a company's "green status" before purchasing online?

I once did an email campaign project for a paper-related company. In the signature line I put "consider the paper industry jobs that might be lost if you do not print this email".

My guess is that most people ignored it. Some commented on it. One student stated that "as president of our organization I could never buy from a company that would use that language."

    I had fun....

You need to start by defining very clearly what the objective of the research is and how the results will be used.

That will determine the methodology you need to use, and in turn the design of the questionnaire. Without a clear understanding of what you're trying to learn, the questionnaire is likely to be "fuzzy" and the methodology may or may not be the right one. The chances you'll get anything useful are slim.

Also, when dealing with Green issues, it makes a difference what kinds of products you're talking about. People who will be very concerned about using plastic bags, for example, will think nothing of burning gallons of gasoline to patronize a restaurant or retail store that are more remote than the neighborhood counterparts.

So you need to be very clear about what specifically you are trying to learn/measure.
