Open-ended Questions As Opposed To Closed-ended Questions

I am brain storming on configuring some open-ended question to referral sources, which would invite the source to engage in conversation. This would allow me to take mental or physical notes about this source, by listening. As opposed to closed-ended questions, which would only invite the referral source to answer in yes or no. I market for Home Health and Medical Equipment. Do you have any suggestions?
