I can't provide examples of times companies have failed in TQM, but I am sure that it has hapenned. Here are some possible reasons:

- focusing so much on finding and making changes that you let your basic business decline

- there is always a cost to change. Whenever you make a change, there are costs to make the change plus in the short run you will often find reduced results.

- not managing the change internally. People generally do not like change. if you don't manage this well such that affected employees are worked through the change process appropriately, people issues could prevent the project from being a success.
