Hippaa Marketing Release Form

I'm working on some marketing materials for a couple of clients in the medical field and we want to use testimonials from current and past patients. To do that, we need to have them sign a release allowing us to use their name, image and other information in our marketing efforts. I want to make sure whatever form we have them sign is legally binding and will protect my clients. Is there a form out there we can use or work off of?

    Because of the nature of this specific use, I would recommend that you not rely on a bunch of marketing folks or a template that may have been designed for a different (or more general) purpose.

    This is a case where you should find a qualified attorney and let him/her create or adapt the release form.

    I am contacting an attorney, but after scouring the internet for some "official" HIPPAA release form I wondered if there was something out there that I just wasn't finding. Thanks.
