    A book was recently published by Guy Kawasaki (chief person at Garage Technologies, a VC firm) called The Art of the Start. I saw him speak and glanced at a copy - and it looks like it would cover much of what you are looking for.

    If you Google "Business Plans", you will find a variety of sites that have templates and sample business plans that you can use as guidelines. Two are:
