I'd like to know what kind of solutions are mid-size websites using for lists management. (Best practices)

My guess is that they are sending the emails from their own servers instead of outsourcing it to low-fee/month companies.

If they are using their own servers, then which software are they using? and how do they make sure that their emails are not being filtered by anti-spam software?

If they are not using their own servers, then what's the name of the companies they choose to outsource this task?

It would be a lot more expensive to do it yourself than to outsource. Try looking at some past questions as this has been asked before a few times.

Also you might want to have a look at the new law in some united states that make people pay a tax for every email they send, it is only a faction of a cent for each email. It is designed to stop spammer who send thousands of emails every second.

I suggest you hire a company to do it for you, they have the skills, they do it everyday day in and day out.

Correction to Carl's post:
