Examples Of A Licensing Rights Agreement, Please!

I am in the process of making a video recording with a partner company. We are going to be interviewing their CEO and recording the interview as a video series. Is there a usual contract or set of documents that must be signed so that the recording can be distributed as a video over the internet, podcast files and PDF transcript?

I have heard several names thrown around like: Media Release, Licensing Rights Agreement, "Grant of Right", "Right to Sub-license" but have had a tough time locating examples of these.

Usually a lawyer is involved in these matters, but I want to have some examples or an online source to create these without having to spend tens of thousands of dollars consulting a lawyer.

Can someone point me in the right direction and offer some sources besides the obvious like Legal Zoom.

    The link that you sent refers to a Press Release (aka Media Release) which is more of a Public Relations piece. What I am looking for is a form that the interviewee will sign to release rights to distribute the content in the video that we are going to record.
